Nomination Form & Guidelines

The nomination process is easy. Simply provide the following information:

Name of nominee
Position or title
Mailing address of nominee (or nearest relative if nominee is deceased)
Email address of nominee (or nearest relative if nominee is deceased)
Phone number of nominee
Years of attendance in Xenia Schools
Name of nominator
Phone number of nominator
Relationship to nominee
Briefly and specifically state in what way or for what reasons you feel the accomplishments or achievements of this individual warrant their inclusion in the Hall of Honor
Include any additional information you may know about the candidate. For example, academic achievement, athletic achievement, outstanding citizenship, and any outstanding service to the Xenia Community Schools or to the students they serve.
List any previous awards or honors of which you may be aware that this person may have received.
List any additional information you may have.

Note: Self-nominations are acceptable as well as posthumous nominations.

Nominations can be submitted at any time throughout the year. Please submit them to the Xenia Area Chamber of Commerce.
